Martha's world:
Assuming the events of DW S3 took place in May 2008 - Martha received her degree in mid-June 2008 and was imprisoned at the beginning of July 2008. Jack arranged her release at the very beginning of 2009 (which is why she didn't know about the Titanic nearly crashing into Buckingham Palace at Christmas when Christina brought it up).
In terms of Torchwood timelines, Martha entered between Kiss Kiss, Bang Bang and Sleeper (circa January 2009). She discovered the window in between Sleeper and To The Last Man (Feb. 2009 in her timeline). Everything ran as normal up till Reset - Martha was absent from Cardiff after the events of Meat (April 2009). Torchwood investigated the Pharm on their own, but Owen didn't get killed - therefore, Jack didn't inadvertently unleash Death upon Cardiff by using the resurrection gauntlet. Gwen did get married, obviously, and she found out about Flat Holm Island. The events of Fragments and Exit Wounds didn't happen.
Doctor Who S4: The Doctor found out about the attempted Sontaran invasion of Earth another way and stopped it (circa June 2009). The events of Planet of the Dead did happen at Easter (Jack mentioned Christina being a wanted woman to Martha). The Stolen Earth/Journey's End happened sometime around October 2009 (but after Martha visited Cardiff briefly at the beginning of the month). Tosh was integral to both protecting the Hub from the Daleks and helping the companions on Earth contact the Doctor.
Meanwhile, in Cittagazze:
January 2012: Martha arrives at the end of the month and quickly becomes one of the Company's physicians.
February 2012: Martha visits Sabriel's world, lending her assistance as a field medic for the battle against the Dead. She and Christina go to Nikola's world (Victorian-era London) at the end of the month.
March 2012: Martha returns home and attempts to recruit a few people from Cittagazze to help with the space whale (the events of Meat).
April/May 2012: Martha doesn't do much at all (though she does meet any number of people at the Observatory).
June 2012: Martha visits Narcissa's world (1970s-era HP!verse; the window is at Hogwarts), then returns to Belisaere for Sabriel's wedding.
July 2012: Martha attends the Fourth of July party in Cittagazze (also Steve's birthday party), visits Raven's world so he can stop her nightmares, and goes to another party in Cittagazze to celebrate the opening of the one hundredth window.
August 2012: Martha returns to Cardiff briefly, summoning a few people to visit and have drinks. She then joins a team to explore an uninhabited world and ends up barely escaping alive - Collette gives her morphing powers after she's impregnated by a facehugger alien. After that, she helps to deal with Weeping Angels in Cittagazze, first finding a note that Helen had left in the past, then returning to her own world with Anakin to fix Jack's vortex manipulator and bring it back to Cittagazze.
September 2012: She goes back to Cardiff again to acquire some morphs, including Torchwood's pet pteranodon. Shortly after returning to the Observatory, she then ventures to Sinnoh to acquire more morphs - she also makes a new friend in Storm, a Shinx that adopts her. Then it's off to Asgard in Meja's world for Ragnarok. At the very end of the month, her nightmares return, and she ends up confessing everything to Helen.
October 2012: Martha falls victim to one of Gabriel's pranks and ends up in a swimming pool full of chocolate syrup. Not much else happens.
November 2012: Martha throws a Bonfire Night party on the beach. She joins another mission to an empty world, because those always end well, and they barely escape after a dead spot wipes out the window (just before the entire world dies). She then becomes inadvertently responsible for Luke putting on an enchanted coat and turning into a beast, and spends a couple days researching in the library before discovering that she has to kiss him to break the spell (which she does). She's also stalked by a dog on the beach (Marco in Irish Setter morph).
December 2012: Martha meets Donna and has no idea who she is.
January 2013: Martha returns to Cardiff for the first couple of weeks, and brings Helen, Luke, and Christina to visit. She then attends a festival on Spira, where she wins a dressphere in a raffle.
February 2013: Towards the end of the month, Martha makes the brilliant decision to visit Donna's world to spy on her family there. Things do not go as planned.
